What is the iCanChange™ - Lifestyle Management Program for Bariatric Surgery?
In the iCanChange™ program, bariatric surgery candidates will work toward adopting healthy lifestyle habits essential for ensuring successful weight loss and maintenance following surgery. Bariatric surgery is on the rise. The years between 1998 and 2002 showed an increase of 400 percent in surgeries performed in the U.S., and this growth is continuing today. Healthy habits, including eating well and increasing physical activity should be adopted before bariatric surgery and continued for life to optimize surgery outcomes.
Why is this program so important for those seeking bariatric surgery?
Bariatric surgery candidates in the iCanChange™ program will gain an understanding of healthy eating habits specifically related to bariatric surgery and understand the importance of ongoing physical activity. The program will also be able to:
- Identify those people who are good candidates for surgery and optimize clinical outcomes of the surgery.
- Identify those people who are poor candidates because of an inability to make needed lifestyle behavior changes and divert them from surgery.
- Divert those people to non-surgical treatment for morbid obesity in an appropriate, clinically sound manner.
We use our dedicated Health Coaching approach to ensure program success.
iCanChange™ participants will receive coaching from the same Health Coach throughout their program, building a trusting relationship with and gaining understanding from a consistent health expert. Leade Health requires its Health Coaches working with bariatric surgery patients to have solid clinical credentials as well as unique coaching skills to create the most effective and supportive environment in which to kindle behavior change. Our Health Coaches are trained in all aspects of bariatric surgery, including understanding surgical procedures, complications, and post-surgery diet progression, as well as the importance of supplementation and support. Our training program was developed with a knowledgeable and experienced team including a physician, registered dietitians, health coaches, and a successful bariatric surgery patient.
We offer the most intensive call model for bariatric participants.
Leade Health is recommending maximum support through our iCanChange™ program for pre-bariatric surgery candidates by offering a coaching call model for high-risk groups with multiple risk factors, which includes 11 calls over a 12-month period. The greatest concentration of calls is within the first 8 weeks of enrollment to educate participants about the necessary lifestyle changes and to allow participants time to establish and practice these new habits prior to surgery.
Coach interactions can be categorized into three types of calls:
- Assessment and Triage (including depression and anxiety screening)
- In-term (7 proactive calls identifying barriers that evoke unhealthy eating habits and redefining new behavior patterns)
- Long-term Follow-up (to ensure maintenance of newly established behavior changes)
We also offer aggregate Outcome Reports that show participants progress in our programs.
Using our proprietary data system based on best-practices in health plan reporting, we provide organizations with comprehensive outcome reports and benchmarking data from our 15 years of experience.
